Traditional compounding pharmacies, which prepare medications in response to individual patient prescriptions, are regulated primarily by state pharmacy boards rather than the FDA. Outsourcing facilities, which may produce larger quantities of compounded preparations without patient-specific prescriptions, are subject to FDA oversight under current good manufacturing practice standards.
The practical implication for consumers is significant: the vast majority of compounding pharmacies in the United States operate under state regulation that varies considerably in rigor and enforcement capacity. A compounded cream prepared at a pharmacy in one state may be subject to very different quality standards than an identical preparation made across a state line. Compounded medications serve a genuine and sometimes irreplaceable function in American healthcare.
